What you will do

• Conduct comprehensive Care Act assessments to identify individual needs and develop tailored support plans.

• Support carers and promote the wellbeing and independence of adults within the community.

• Respond to urgent situations as part of a supportive duty rota, assessing needs and providing immediate assistance where required.

• Undertake safeguarding work to protect adults at risk, ensuring their wellbeing and safety.

• Apply the principles of the Mental Capacity Act confidently, carrying out risk assessments and positive risk management to help individuals achieve their goals.

• Manage a diverse caseload, prioritising effectively to meet the varying needs of the adults you support.
