What you will do:

• Undertake urgent community nursing assessments for adults referred to the Rapid Response Service.
• Deliver timely clinical interventions aimed at preventing unnecessary hospital admissions and facilitating early discharge from acute services.
• Assess, plan, implement and evaluate care for patients presenting with acute illness, deterioration of long-term conditions and complex healthcare needs.
• Monitor patient condition, recognise clinical deterioration and initiate appropriate escalation pathways when required.
• Provide clinical advice, support and education to patients, carers and support networks to promote self-management and independence.

Person Specification

Qualifications

• Registered Adult Nurse qualification.
• Current registration with the Nursing and Midwifery Council (NMC).
• Evidence of continued professional development relevant to urgent community nursing practice.
• Community Nursing qualification, Acute Care qualification or equivalent experience desirable.

Experience

• Post-registration experience as a Registered General Nurse within community, urgent care, acute medical or integrated care services.
• Experience undertaking autonomous clinical assessments and managing complex patients within community settings.
