The Killingholme Surgery is looking for an experienced and motivated Advanced Nurse Practitioner to join our small practice team.

This is a varied, hands-on role involving the assessment and management of patients with acute and long-term conditions, alongside chronic disease management, QOF, routine nursing services and general clinical duties.

As a small practice, we need someone who is confident working independently, able to make clinical decisions within their scope of practice and comfortable managing a broad range of patient needs. The successful candidate will need to be organised, flexible and willing to take responsibility for their own workload, including clinical reviews, investigations, results and follow-up.

The role offers an excellent opportunity for an ANP who enjoys autonomy, continuity of care and being an important part of a small team, working closely with the GP and wider practice staff.

  • Make safe, evidence-based prescribing decisions, taking account of the patient's clinical presentation, medical history, current medication and relevant investigation results.
  • Arrange appropriate monitoring and follow-up for prescribed medicines.
  • Maintain accurate records of prescribing decisions and ensure appropriate safety-netting and review. 30 hrs per week Monday, Thursday and Friday
  • Main duties of the job

  • Assess, diagnose and manage acute illnesses, minor injuries and ongoing health conditions, including examinations, treatment plans, investigations, referrals and safety-netting.
  • Prescribe as an independent prescriber within competence and scope of practice.
  • Manage chronic disease reviews, including diabetes, asthma, COPD and hypertension.
  • Support QOF achievement through recalls, reviews, accurate coding, identifying gaps and following up outstanding actions.
  • Undertake cervical screening, vaccinations, injections, venepuncture and other nursing procedures.
  • Assess and manage wounds, perform dressings and remove sutures or clips where trained and competent.
  • Undertake structured medication reviews and medicines optimisation.
  • Review blood results, arrange appropriate follow-up and escalate abnormal results to the GP where necessary.
  • Maintain accurate clinical records and ensure clinical tasks are completed.
  • Provide health promotion and preventative care advice.
  • Work independently while collaborating with the GP and practice team, escalating concerns when required.
  • Maintain professional registration, competencies and mandatory training, and comply with safeguarding, confidentiality, infection control and clinical governance requirements.
  • Participate in audits, significant event reviews and service improvement.
  • Undertake other duties appropriate to the role and level of competence.
