Job summary
Due to retirement, we are seeking an exceptional clinical pharmacist to support the practice team and patients.

Main duties of the job
- Conducting comprehensive medication reviews and optimising medication regimens
- Provide advice on medication management to patients, healthcare professionals, and the multidisciplinary team.
- Support the management of chronic diseases through medication adjustments and monitoring after discussion with medical team.
- Corresponding with secondary teams if clarification needed of prescribing plans.
- Discharge medication reconciliation.
- Support health board prescribing incentive schemes, and clinical audits.
