Stroke Association Support Coordinator

Stroke Association Support Coordinator Ref: REF000058 Location: Home-based, Cardiff and Vale. This role requires frequent travel across a large geographical locality to visit people at home and in community settings in the Cardiff and Vale area. Travel will also be required for team meetings and other work-related meetings Salary: Circa £17,000 per annum Hours: 21 hours per week Contract: Permanent Our services are contracted; we currently have funding for this contract until October 2027. We are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join the Stroke Recovery Team in Cardiff and the Vale, Wales. Supporting stroke survivors and families in the community and in a group setting with their stroke recovery, rebuilding their lives and rebuilding confidence. Reporting to the Service Delivery Coach in Wales, you will have the ability to support Stroke Survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.
