Main Purpose of the job: The purpose of the Work and Heath Coach role is to provide personalised coaching and support to people who have signed up to WorkWell service within Lewisham Works. This support aims to empower participants take steps towards returning to work, or to stay in work, while managing a physical or mental health condition. _______________________________________________________________ Summary of Responsibilities and Personal Duties: The main duty of the role is to provide one-to-one personalised wellbeing and employability coaching and support to Lewisham residents on the Lewisham Works WorkWell programme participants. Appointments are delivered face-to-face, online, or via telephone and could involve: Assessing a persons employability and wellbeing, using such tools as the EQ5DL Creating personalised action plans with participants, Providing referrals and/or signposting to other suitable support services, Providing health and wellbeing advice, Providing career guidance, Offering employment advice (e.g. CVs, job search, interview techniques), Helping participants identify workplace adjustments to support health needs, Carrying out health and wellbeing assessments. You will be responsible for managing your own caseload of participants to meet performance targets and service level agreements of the WorkWell programme. Additional tasks Working closely with primary care and other health partners to support the integration of employment support into health services. This will include working from within partner settings autonomously and contributing ideas to the ongoing development of this integration. Building relationships with other services to enable cross referrals that will support residents to maintain employment and wellbeing. Maintaining up to date information and resources to support residents wellbeing and employability. This role is part of the Lewisham Works service and the health and work coach must keep abreast of developments in the wider service and external employability landscape to ensure an effective system of support for residents. Generic responsibilities To represent the Council at meetings and events to promote employment and skills support To adhere to the Councils safeguarding policies and procedures To maintain appropriate monitoring systems to track interactions and clients outcomes into jobs, training and work experience places. To provide support in the planning and delivery of events To assist in the preparation of external funding bids and project proposals. To promote Equality of, and access to services at all times in line with the Councils Equal Opportunities policy. To ensure that the operation of the scheme complies with all relevant statutory legislation To undertake appropriate personal development in line with departmental needs and strategic plans, commensurate with the grade and scope of the post To work toward the achievement of objectives / tasks mutually agreed and included within the individual and departmental work plans To undertake any other duties commensurate with the grade and scope of the post as directed by the line manager To be available to attend occasional weekend and evening events and /or meetings Assist in carrying out the Council's environmental policy within the day to day activities of the post. Internal Contacts: These include Economy, Jobs and Partnerships Team, Baseline, Housing, Adult Social Care, Public Health External Contacts: This will include Integrated Care Board, Primary Care, Employers and local businesses, Job Centre Plus, College and FE Providers, advice agencies, wellbeing services To carry out the duties of the post with due regard to the Councils relevant policies, codes and procedures. To carry out duties with due regard to the Councils values and behaviours All employees are required to participate in Appraisal and Performance Management processes and to undertake appropriate training and development, including mandatory induction training, identified to enhance their work. Undertake other duties, commensurate with the grade, as may reasonably be required. Consideration will be given to restructuring the duties of this post for a disabled postholder.
