What you will do

• Collaborate closely with health colleagues, monitoring long-term CHC provision and ensuring quality service delivery.
• Complete joint reviews and assessments in line with the Care Act 2014, ensuring support is tailored to meet individual needs.
• Engage in Decision Support Tool Meetings to facilitate appropriate care planning.
• Participate in Safeguarding and Court of Protection work to uphold the rights and interests of adults within the community.
• Conduct thorough assessments and evaluations to support decision-making and care provision.
