What you will do

• Undertake strengths-based assessments, reviews, and care planning for adults with complex mental health needs in accordance with the Care Act 2014.
• Manage a diverse caseload within community and hospital settings, ensuring timely interventions and effective risk management.
• Complete safeguarding enquiries and work to protect vulnerable adults from abuse or neglect.
• Collaborate with NHS professionals, Approved Mental Health Professionals (AMHPs), community mental health teams, housing providers, and voluntary sector partners.
• Participate in Mental Health Act assessments and contribute to statutory mental health functions when required.

Person Specification

Qualifications

• Degree or equivalent qualification in Social Work.
• Successful completion of Assessed and Supported Year in Employment (ASYE) desirable.
• Advanced post-qualifying development in adult mental health practice is advantageous.

Registration and Compliance

• Current registration with Social Work England (SWE).

Experience

• Experience working within Adult Social Care or Community Mental Health Services.
• Demonstrated ability to manage complex mental health cases and safeguarding concerns.
