What you will do:

• Manage a caseload of service users, supporting them through the entire journey from referral, assessment and treatment to sustained recovery and discharge planning.
• Conduct thorough assessments, risk evaluations, and create personalised recovery plans tailored to meet individual needs.
• Deliver evidence-based psychosocial interventions, one-to-one key working sessions, and structured group programmes to support service users effectively.
• Collaborate with prescribing clinicians and healthcare professionals to enhance treatment plans and improve overall health outcomes.
• Promote harm reduction initiatives, focusing on overdose prevention, safeguarding awareness, and health promotion activities.

Roles and Responsibilities:

• You will remain at the forefront of delivering comprehensive assessments, managing risk, and maintaining detailed recovery planning, always ensuring interventions meet the unique needs of each individual.
• You will leverage your experience to manage challenging behaviours and support vulnerable adults, working within community services to drive positive change.

Person Specification:

Qualifications:

• Educated to Level 3 standard or equivalent in Health & Social Care, Substance Misuse, Community Work, or a related discipline.
• Professional development in motivational interviewing, recovery-focused practice, safeguarding, or mental health interventions is a definite plus.

Experience:

• Prior experience with individuals affected by drug and alcohol misuse, mental health needs, homelessness, or social exclusion is required.
• Demonstrated experience in managing challenging behaviours and supporting vulnerable adults within community services.
