• Conduct comprehensive Care Act Assessments and Mental Capacity Assessments.
• Engage in effective Case Management and Support Planning.
• Work collaboratively with multidisciplinary professionals and partner agencies.
• Undertake Duty Work with responsibility, professionalism, and integrity.
• Manage a caseload effectively, providing personalised and outcome-focused solutions.
• Liaise with other professionals to ensure a cohesive and coordinated support network.
• Address complex situations with a calm, solution-focused approach.
• Support individuals in achieving greater independence, wellbeing, and positive outcomes.
