What you will do:

• Conduct Care Act assessments to determine individuals' needs and appropriate care and support.
• Manage safeguarding cases and work on risk management plans for those with complex mental health needs.
• Fulfil Mental Health Act duties, including Section 117 aftercare arrangements.
• Collaborate with multi-disciplinary teams, including health partners, to deliver holistic care and support.
• Make autonomous, informed decisions on complex cases to improve service user outcomes.
• Maintain comprehensive records and ensure all documentation meets statutory requirements.
