Rochdale Borough Council is excited to offer an ongoing opportunity for an experienced Adults Social Worker to join their dynamic Community Team on a locum basis, filling in during maternity leave. This role is ideal for those who thrive on strengths-based practice, enjoy fostering independence for service users, and relish collaboration with individuals, families, carers, and partner agencies. You will handle a diverse adult social care caseload, complete assessments, support planning, and perform reviews in line with the Care Act 2014. Additionally, you will play a critical role in supporting hospital discharge pathways, working with the Home First Hospital Discharge Hub during peak periods.

What you will do:
- Manage a caseload of adults with varied care and support needs.
- Conduct Care Act assessments, reviews, and design person-centred support plans.
- Encourage independence, wellbeing, and choice for service users and carers.
- Utilise community resources and voluntary services to propose creative solutions.
- Perform Mental Capacity Assessments and complete Section 42 Safeguarding Enquiries.
- Participate in the duty rota, providing prompt responses to urgent cases.
- Facilitate hospital discharges via the Home First Hospital Discharge Hub when necessary.
- Collaborate with health professionals, partner agencies, and internal teams.
- Offer advice, guidance, and support to colleagues, service users, and carers.
- Maintain detailed case records using electronic systems such as Liquidlogic.
Requirements:
- Social Work England Registration: Must be registered with Social Work England.
