We’re hiring for several roles at Live Well Wakefield, a community-based service that supports adults aged 18+ to improve their health, wellbeing and independence. We’re looking for dynamic, compassionate and motivated people to join the team. About the roles As a Social Prescriber, you’ll support people one-to-one to identify their needs, set wellbeing goals, and access community-based solutions. The roles we have available are: · 1 link worker within the Primary Care Network · 1 social prescriber within our Trailblazer work (employment support) · 2 social prescribers within our Healthy Working Life project (employment support) All roles are fixed term until 31 March 2027 or 30 June 2027.
