We have an exciting opportunity for a Care Coordinator to join our Team in Eastbourne. This fantastic opportunity is ideal for an individual with experience as a Domiciliary Coordinator or similar role, with a good knowledge and understanding of healthcare. What we are looking for: Under the direction of the Registered Manager, you will be responsible for ensuring the compliant delivery of outstanding care for our Clients in Eastbourne, completing client/staff rotas to ensure we meet our client needs. The successful candidate will also be able to competently carry out client assessments and complete person centred care plans ensuring they meet CQC requirments. You will also undertake care work in the community if required and be responsible for holding the On–call phone on a rotational basis as required. Experience in care is essential and experience as a Care Coordinator would be extremely desirable & advantageous. You must be a people person with excellent communication skills to support our team of Carers.
