What you will do

• Conduct assessments and develop personalised transition plans for young people preparing for adulthood.
• Facilitate and coordinate multidisciplinary meetings to ensure cohesive care and support planning.
• Work closely with families, carers, and professionals to provide holistic support.
• Advocate for young people's rights, ensuring their voices are heard throughout the transition process.
• Monitor and review cases to ensure plans are implemented effectively and provide ongoing support where required.
