• Lead the clinical management of care packages for individuals using our services.
• Coordinate and monitor the delivery of person-centred healthcare through complex clinical decision-making.
• Deliver individual and group therapy, utilizing CBT skills.
• Collaborate with the MDT to assess and plan care, responding effectively to changes in health.
• Provide necessary physical care, respecting individuality and supporting clients' personal goals.
• Ensure effective documentation and communication in line with policies and procedures.
• Lead the development and maintenance of service standards, promoting continuous improvement.
