As a Family Support Worker, you will be involved in exciting projects stemming from the Families First reforms. Your role is crucial in providing statutory help and support to children, ensuring their needs are met while contributing to a vital pilot programme that tests innovative service delivery models.
What you will do:
- Complete assessments of children's care and support needs
- Develop care plans to ensure children's needs are addressed adequately
- Chair reviews of paid services and care plans for active monitoring and improvements
- Work in partnership with parents, carers, children, and multi-agency professionals to deliver comprehensive support
- Complete Social Care Advice for children undergoing an EHCP assessment
