What you will do:

• Manage a social work caseload, contributing significantly to weekly peer reviews by using an asset-based, community-led approach.
• Nurture and foster constructive relationships with individuals, their families, and the community, promoting well-being and support.
• Conduct comprehensive assessments, support planning, and reviews for individuals receiving care, focusing on the principles of the Care Act.
• Collaborate with colleagues from your own service as well as other council services and external agencies to gather relevant information for Assessment and Support Planning activities.
