What you will do:
- Work collaboratively as part of a supportive Multi-Disciplinary Team (MDT) to plan, monitor, and deliver interventions that support adults and carers.
- Manage social care cases with a focus on personalisation and inclusion, ensuring compliance with the Care Act 2014.
- Conduct needs-led assessments, develop care and support plans, and participate in multidisciplinary assessments where required.
- Support adults at risk by participating in safeguarding processes, Multi Agency Safeguarding Hub (MASH) activities, and other inter-agency collaborations.
- Provide guidance, advice, and supervision to ASYE/newly qualified social workers and Care Navigators.
- Engage actively in community forums to advocate for positive social work practice and the needs of vulnerable adults.
- Author and present complex assessments and care planning information, effectively using information and communication technology.
- Chair strategy meetings and professional case conferences, ensuring a holistic and person-centred approach to care.
- Ensure compliance with corporate policies and maintain the highest standards of professional conduct.
