What you will do:

• Assess and manage a caseload of children and young people with disabilities to improve their well-being.
• Work collaboratively with families, carers, and external partners to provide high-quality, person-centred support.
• Participate in multidisciplinary meetings to ensure the best outcomes for young people.
• Develop intervention plans and monitor progress to achieve agreed objectives.
• Provide expert advice and guidance to less experienced team members.
