What you will do
- Care Act Assessments: Conduct assessments in line with the Care Act 2014 to identify needs and develop tailored care plans.
- Aftercare Responsibilities: Undertake Section 117 Aftercare duties to support recovery and long-term wellbeing.
- Safeguarding: Carry out Adult Safeguarding work to protect vulnerable individuals.
- Strengths-Based Practice: Promote independence through strengths-based approaches and personalised interventions.
- Multidisciplinary Working: Participate in multi-agency meetings with health and social care professionals.
- Service Delivery: Provide high-quality, person-centred support to adults with mental health needs.
Qualifications / Requirements
- Qualified Social Worker with Social Work England registration.
- Experience within Adult Social Care, ideally within a CMHT setting.
- Strong knowledge of the Care Act 2014 and safeguarding procedures.
- Experience supporting adults with mental health needs.
- Ability to work effectively within multidisciplinary teams.
- Excellent communication, assessment, and organisational skills.
