What you will do:

• Undertake comprehensive Care Act assessments, identifying eligible needs and promoting independence through strengths-based approaches.
• Develop, implement, and review care and support plans in collaboration with service users, carers, and partner agencies.
• Manage a diverse caseload, including complex cases involving risk, crisis intervention, and fluctuating needs.
• Lead and contribute to safeguarding adults enquiries (Section 42), ensuring timely and proportional responses to risk.

Person Specification:

• Degree in Social Work or an equivalent recognised qualification (e.g., DipSW/CQSW).
• Registered with Social Work England.
• Sound knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding frameworks.
• Understanding of the Equality Act 2010 and Making Safeguarding Personal principles.
