What you will do:

• Manage a varied caseload, providing tailored support to adults with mental health needs.
• Conduct comprehensive assessments to understand the individual needs of older people.
• Develop and implement care plans to ensure effective support and intervention.
• Collaborate with partner agencies to provide a multidisciplinary approach to care.
• Undertake safeguarding work to protect vulnerable adults and uphold their rights.
• Maintain accurate and detailed records of your work and client interactions.

Qualifications / Requirements:

• Registration with Social Work England.
• Proven experience in Adult Social Care.
• Strong understanding of Older People's Mental Health.
• Exceptional assessment and case management skills.
• Ability to work collaboratively within a multidisciplinary environment.
