What you will do:

• Manage a caseload of up to 35 patients, providing specialised care for those with severe and enduring mental health needs.
• Develop and implement care plans tailored to each patient’s unique situation, promoting recovery and wellbeing.
• Engage collaboratively with healthcare professionals, family members, and the wider support network to ensure comprehensive care delivery.
• Regularly assess and review treatment progress, adapting strategies as necessary to meet evolving needs.
• Maintain meticulous documentation of patient interactions and progress according to professional and regulatory standards.

Requirements:

• Must be registered with the HCPC, demonstrating professional competence and adherence to standards.
• A valid driving licence is essential, as you will be visiting patients within their communities across Loughton.
