If yes, this is your chance to join our Health and Social Care Partnership as a Social Care Officer, in the Supported Living Team, within Perth and Kinross.  Experience is not necessary as all training will be provided. Our team provide supportive care to vulnerable service users with varying learning disabilities and mental health to allow them to live independently in their own homes and their community. As a successful post holder, you will be responsible for: Following a planned schedule of service user visits. Supporting service users to maintain their own home. Supporting service users based on their assessed needs. Supporting service users with personal care tasks when required to enable the person to reach their optimum level of independence. Monitoring health & wellbeing and supporting to appointments. Supporting service users to access community facilities / activities. Supporting service users to recognise and achieve their potential. Protecting individuals from risk or abuse and to report any concerns through appropriate process. Ensuing Health & Safety for yourself and service users in line with PKC policy and procedures. Participating in assessments. Maintaining accurate records and reporting on service users' progress. Complying with the National Care Standards and SSSC Codes of Practice. Please note, we are currently unable to offer sponsorship for this post.Skills & Experience Required You will be required to register with the Scottish Social Services Council (SSSC) within the required timescales. As part of this registration commitment, you will undertake your own personal development and work towards recognised qualification if required. You must have a valid UK driving licence (manual) as this post requires you to travel to a variety of locations across Perth and Kinross and is required for service user transport and accessing rural locations. Ability to work a flexible rota, including alternate weekends.
