What you will do:

• Manage incoming referrals and assess the needs of adults aged 18 and over, addressing issues such as physical disabilities, age-related frailty, substance misuse, and domestic abuse.
• Complete thorough Care Act assessments and conduct regular reviews to ensure the best outcomes for residents.
• Undertake safeguarding work to protect vulnerable adults in the community.
• Operate from the office within a supportive and experienced team, handling complex cases effectively.

• A qualified and experienced Adult Social Worker with a strong understanding of Care Act assessments and safeguarding processes.
• Capable of working with complex cases and collaborating with multi-agency partners.
• Must hold current registration with Social Work England.
