What you will do:

• Lead and oversee the Hospital Discharge and Post-Discharge Assessment Team, ensuring that all interventions are timely and of high quality.
• Support the seamless transition of patients from hospital to community settings.
• Provide guidance and supervision to team members, enhancing their professional capabilities.
• Liaise with healthcare professionals and community services to ensure a coordinated and efficient discharge process.
• Conduct regular reviews and assessments to maintain service excellence.
• Maintain accurate and comprehensive records in compliance with Social Work England registration requirements.
