What you will do:

• Undertake Care Act 2014 assessments, reviews, and support planning using a strengths-based, person-centred approach.
• Manage a complex caseload of adults with various health-related needs, including long-term conditions, frailty, mental health, and safeguarding concerns.
• Carry out Section 42 safeguarding enquiries and contribute to risk management and protection planning in line with Lambeth and London multi-agency procedures.
• Apply the Mental Capacity Act 2005, including conducting capacity assessments and making best-interest decisions, as well as engaging in Deprivation of Liberty Safeguards when required.

Qualifications and requirements:

• Degree or postgraduate qualification in Social Work or an equivalent UK-recognised qualification.
• Current registration with Social Work England.
• Sound working knowledge of the Care Act 2014, Mental Capacity Act 2005, and adult safeguarding frameworks.
