• Support young people leaving care by developing and implementing personalised pathways to independence.
• Collaborate with colleagues in the Leaving Care Team to ensure a cohesive approach to transitional care planning.

• Monitor and review the progress of young people, adjusting plans as necessary to meet their changing needs.
