What you will do:
- Conduct timely and proportionate assessments and reviews in accordance with the Care Act for residents living in Extra Care Housing.
- Guide and support residents through the transition process, ensuring their care and support plans are updated to reflect the latest accommodation changes.
- Collaborate effectively with housing providers, incoming and outgoing care providers, commissioners, and health professionals to maintain continuity of care.
- Identify potential risks arising from changes in care provision and accommodations, implementing appropriate risk management and safeguarding measures.
- Perform Mental Capacity Assessments and participate in Best Interest decision-making processes when necessary.
- Keep residents, their families, and advocates informed and supported throughout the transition process.
- Maintain accurate and timely case records, adhering to statutory and organisational standards.
Essential Requirements:
- Qualified Social Worker with registration from Social Work England.
- Recent experience in adult social care, particularly in settings dealing with older people or housing-based support services.
- Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding responsibilities.
