What you will do:
- Conduct assessments of adults to determine their needs and develop comprehensive care plans.
- Work collaboratively with service users their families and a multi-disciplinary team to ensure effective delivery of services.
- Act as an advocate for individuals ensuring they have access to the services and support they need.
- Maintain accurate and complete records of care and services provided adhering to Social Services regulations.
- Participate in case reviews and contribute to the development of service improvement initiatives.
Requirements of the Social Worker:
- A degree or equivalent in social work.
- Current registration with Social Work England.
- Substantial frontline experience within Adults Social Services in the UK working as a qualified Social Worker is essential.
