We are seeking caring and dedicated Outreach Support Workers to join Leeds Autism Services, supporting adults with autism to live fulfilling and independent lives.
Position: Outreach Support Worker
Salary: £13.45 per hour
Location: Leeds, West Yorkshire
Hours: Full time and part time hours available, including flexible and unsocial hours
Contract: Permanent

About the role
As an Outreach Support Worker, you will help to improve the lives of the adults we support by promoting understanding and acceptance of people with autism. You will support individuals to be included and feel valued within their communities, while enabling them to make choices and decisions about their own lives.
Key responsibilities include:
Ensuring the welfare and development of service users by providing the highest standards of care
Supporting person centred care planning and helping individuals achieve personal goals and outcomes
Promoting inclusion, independence, choice and decision making
Supporting service users in a non judgemental way in line with Positive Behaviour Support principles
Supporting access to activities that help individuals achieve their goals
Facilitating communication using each individual's preferred methods
Maintaining accurate records including care plans, health and safety and medication
Administering and storing medication in line with policies and procedures
Recognising changes in wellbeing and reporting concerns appropriately
Maintaining professional boundaries at all times
You will work across community settings, service users' homes and different organisational bases, with flexibility required for changing rotas and activities.

You will be a compassionate and reliable individual with a genuine commitment to supporting people with autism.
Essential skills and experience:
Ability to empathise and advocate for people with autism and or learning disabilities
Good communication skills, both written and verbal
Ability to work as part of a team
Basic IT skills including email and Word
A positive, flexible and non judgemental approach
Strong interpersonal skills and commitment to equality and diversity
Desirable:
NVQ Level 2 or 3 in Care or equivalent
Experience supporting people with autism, learning disabilities or challenging behaviour
Experience of person centred planning
Ability to drive and access to a vehicle
